SSTP 2010 On-line Alumni Survey

I always hate filling out anything that asks for my name for fear of ending up on another mailing list.  Let me assure you, the Name and address section is for my data base ONLY.  It ensures that we are not getting multiple surveys from one participant.

To ensure your privacy, Name/Address data will be stored separate from the survey data.

The Survey Data itself will be used for internal program evaluation and to report outcomes to potential program funding sources.
