1. Print out this form (DOWNLOAD THE FORM) (late July and August Form -NEW)

2. Fill out the entire registration form indicating your choice of workshop(s). Do not submit the form via e-mail. We must have payment in order to process your registration.

3. Submit your completed form, using one of the following methods:

• Fax the form, with credit card information included, to 319-335-4743.
